• Frequently assist with discharge planning only
• Attend doctor’s office’s to have OCF 19 forms filled in by the physician
• Ensure appropriate services are secured and referrals made
• Implementation of recommendations made by physicians or in specialist reports
• Review of file and recommend services or assessments required
